Terms of Service
By booking and attending appointments, you agree to the following terms and conditions:
1. Purpose of Services - I provide occupational therapy and rehabilitation services in accordance with AHPRA and Occupational Therapy Board of Australia's professional standards. These services aim to help you increase your functional capacity in daily activities, work and meaningful life roles. I also offer broader capacity building and health coaching services that draw on my occupational therapy clinical experience, behaviour-change principles and neuroscience-informed strategies. These coaching sessions are non clinical and are designed to complement, not replace, any medical or allied-health treatment provided by your existing care team.
2. Collection and Use of Personal Information - To support your care and comply with health provider requirements, I collect personal and relevant health information necessary for assessment, treatment, reporting, and communication with your insurer and healthcare team. This may include your contact details, medical history (only where relevant), prior referrals and assessments, progress reports, and session notes. Where further funding is being requested, objective measures and analysis of progress will be required.
3. Informed Consent and Insurance Funding - By engaging in services, you provide informed consent for the collection, use, and secure storage of your personal information for clinical, administrative, and insurance funding purposes. This includes sharing relevant therapeutic information with your insurer if required for funding approval and claims processing. You may withdraw consent at any time, understanding this may affect your eligibility for funded services.
4. Reporting and Communication with Insurers - I will provide reports and progress updates to your insurer as required, to support your funding and ongoing care. These reports will include information about your progress, treatment goals, and outcomes. I will ensure your information is shared securely and only as necessary to support your goals and the purpose of the service.
5. Storage and Security - Your information is stored securely online, in an encrypted practice management system (Halaxy). I take all reasonable steps to protect your information from loss, misuse, or unauthorised access. Records are retained for at least 7 years from the last contact,, or until the person turns 25 if they were under 18 at the time of service, in accordance with AHPRA record-keeping requirements for health providers in Australia.
6. Access and Correction - You may request access to your personal information at any time. If something is inaccurate or incomplete, you may request a correction. I will respond within a reasonable timeframe, ideally within 5 business days.
7. Fees, Funding, and Cancellations - Fees for services will be discussed prior to your first appointment and confirmed in writing. If your services are funded by a third party/insurer, your out-of-pocket costs (if any) will be clearly communicated prior to the service commencing. Cancellations with less than 24 hours’ notice may incur a fee of 50% if made on more than one occasion as this time is reserved for you. Additional charges may apply for reports or services required outside of insurer funding agreements.
8. Telehealth Services - Telehealth sessions are delivered through secure platforms such as Google Meet. Please ensure you are in a private, quiet space where you feel comfortable. If technical issues arise, we may need to reschedule or call via phone.
9. Your Role in the Process - Your engagement in each session is vital to your progress. You agree to provide accurate information, communicate openly, and inform me of any changes in your circumstances or health. My role is to guide and support you; your role is to participate in a way that feels safe and sustainable.
10. Feedback and Concerns - If you have feedback or concerns about your care, privacy, or funding, please reach out so we can address it together. If an issue cannot be resolved, you may contact the Office of the Australian Information Commissioner (OAIC) or your insurer’s complaints process.
11. Acceptance of Terms - By booking or attending appointments, completing intake forms and continuing to engage in services, you acknowledge and accept these Terms and Conditions.
